Your RX Benefit may also be used at non-participating pharmacies. Simply pay for the prescription, keep your receipt, and contact Amwins to file a claim for reimbursement.
You have 12 months from date of service to request a reimbursement.
You can fill out the form and submit via email customerservice.amwinsrx@amwins.com or print form and mail to the address on the form with all claim information required. Your claim request will be processed within 30 days.

What is a Grievance
A grievance is an expression of dissatisfaction with the services provided by Amwins Rx or our contracted providers. If you have a concern regarding the services provided by Amwins Rx, we encourage you to call us right away by calling our Amwins Rx Customer Care Department at Tel: 1--800-580-4403.
How to file a Grievance
Concerns can be conveyed telephonically at 1-800-580-4403, FAX 1-866-650-3622 or through correspondence at:

In order to exercise this right, you must file your grievance no later than 60 days after the event or incident that precipitated the grievance.
