Better results from existing medication
Medication Therapy Management (MTM) is a service offered by Amwins Rx that helps members achieve better results from their medications. When MTM services are in place, a specially trained pharmacist will work with the member and their doctor to make sure all questions are answered and the full benefits from the medications are realized.
In order to qualify for these services, members must meet all of the following targeting criteria;
- Have a minimum of 3 of the following 5 chronic conditions
- Diabetes
- Respiratory Disease - Chronic Obstructive Pulmonary Disease (COPD)
- High Blood Pressure
- Bone Disease - Arthritis, Osteoporosis
- Alzheimer's Disease
- Be taking at least 8 unique medications
- Likely to incur medication costs which equal or exceed $4,255 annually
